Support Coordination and Psychosocial Recovery Coaching

If there are challenges in your life that may impact your care, our specialist support coordinators will work with you to overcome these challenges and ensure consistent delivery of care from your support network.

Support Coordination

Our aim is to ensure you are maximising your plan with the support that helps you achieve your goals.

Our support coordinators ensure you have choice, control and flexibility in your support plan. We will assist you to understand and implement your plan to make sure your plan is working to achieve your goals.

 

Our support coordinators will work with you to make sure you have a good balance of services included in your plan which includes any necessary health or medical supports, as well as making time for your hobbies, behaviour supports or spending time in the community.

Our support coordinators will:
 

  • Help you understand your plan.

  • Provide guidance on local support workers and services that best meet your needs.

  • Help negotiate service agreements with providers.

  • Complete reviews of your support plan to ensure the services are meeting your needs.

  • Provide you with information and training to help you better manage your plan.

  • Focus on maintaining control, choice and flexibility.

  • Help you with NDIS plan reviews.

  • Help to resolve any issues or challenges in your plan

We can also provide specialised support coordination for people with more complex needs or support requirements.

Psychosocial Recovery Coaching

Psychosocial disability is a term used to describe a disability that may arise from a mental health issue.

​

Our psychosocial recovery coaches help support participants to take more control of their lives and better manage the complexities of daily living. They work with our participants, their families and support networks, including other service providers to support you with your recovery. Recovery means being able to live a purposeful and meaningful life.

Our recovery coaches all have extensive experience in the mental health sector and will:​

  • Spend time with you, your family, friends and your support network, getting to know you.

  • Gain an understanding of your needs.

  • Provide you with information about appropriate services and programs that may help you.

  • Help you get support from mental health services.

  • Work with you to help you understand the NDIS services you are eligible to receive.
